By signing below, I authorize Woodinville Sports Club (WSC) to receive and share necessary medical information with emergency personnel and healthcare providers in the event of an incident involving my child. I understand that WSC staff are not licensed medical professionals, and I authorize WSC to administer emergency treatment and to arrange emergency transportation as needed. In the case of a medical emergency, WSC will act according to its established protocols and contact emergency services.
I acknowledge that participation in WSC programs involves inherent risks, including but not limited to physical activity, outdoor play, and group interaction. I voluntarily assume all risks associated with my child’s participation and hereby release and hold harmless Woodinville Sports Club, its employees, agents, and volunteers from any liability, claims, or demands arising from injury, illness, or accident that may occur during programming activities, except in cases of gross negligence or willful misconduct.
I also acknowledge that I have read, understood, and agree to abide by all WSC rules, regulations, and policies, including those related to health and safety, behavior, and medication. I understand that if my child is sick or exhibiting symptoms of illness, they must remain at home in accordance with WSC’s health and illness policy to ensure the well-being of all participants and staff. If symptoms of illness arise during programming, I understand that a parent or guardian will be contacted and will be required to pick up the child as soon as possible.
